The Achille's heel of vocabulary learning is lack of repeated recall. Allow 24 hours to elapse before attempting to recall items. Eighty percent of the words that you recall correctly after 24 hours will still be remembered six months later.
Another factor that is prevalent is an inability to make sense of figurative language because the student is not familiar with background information such as the problem that Achilles had with his heel.
